OBRA Coordinator
POSITION: | OBRA Coordinator – Hourly, Full-Time (30-40 hours) |

POSITION SUMMARY
Coordinate the OBRA Program and provide services to persons with developmental disabilities who reside in nursing homes. Establish good working relationships with nursing home providers and other agencies.
QUALIFICATIONS
- Master’s degree in Psychology, Social Work, OR a nursing degree (RN) from an accredited school. Must be licensed in Michigan as a Limited Licensed Psychologist (LLP), Licensed Master Social Worker (LMSW), or hold a full RN license.
- Experience working with persons with intellectual and developmental disabilities, eligible for or is qualified for QIDP designation
- Understanding of OBRA legislation preferred.
- Experience providing clinical services including diagnosis and treatment planning.
- Experience leading a clinical team preferred.
- Ability to perform job duties with minimal supervision.
- Valid Michigan driver's license and personal vehicle.
- Excellent organizational, time management, and communication skills.
- Ability and willingness to work flexible hours.
- Ability to work effectively with a team.
REQUIREMENTS
- Safely operates a personal vehicle independently and, as part of program responsibilities when needed, transports individuals served.
- Effectively communicates in both oral and written formats with persons served, agency staff, external providers, and community partners.
- Maintains focus and active participation during meetings and discussions for extended periods of time.
- Comprehends and accurately follows verbal and written instructions.
- Ability to hear and respond effectively during conversations, telephone communications, and emergency alerts.
- Ability to see and interpret visual information accurately, including depth perception and a sufficient field of vision to perform essential job functions.
- Ability to move independently and safely across various surfaces and within different environments to perform essential job functions.
KEY RESPONSABILITIES
- Lead and coordinate the OBRA program, delegating intakes and ensuring timely completion of individual assessments by team members. Compile and submit final summaries and agency recommendations to MDHHS in compliance with program standards and regulatory requirements.
- Provide ongoing mental health monitoring and support for assigned persons served as needed, ensuring compliance with treatment plans and agency standards.
- Develop and deliver training on OBRA requirements and nursing home placement processes, and represent the agency at meetings, trainings, and speaking engagements as assigned.
- Develop and maintain collaborative relationships with nursing homes, hospitals, and community agencies.
- Provide comprehensive assessments for PAS/ARR referrals.
- Build and maintain ongoing communication to support care coordination.
- Prepare OBRA reports and other documentation as directed by the Director of Community Services.
- Identify and address barriers to nursing home residents being integrated into community services.
- Comply with all policies and standards of Thresholds, Network180, and MDHHS, and complete required documentation accordingly.
- Perform other related duties as assigned by the supervisor.
